fix(npm): add gRPC read/send timeouts to proxy host location blocks

Adds grpc_read_timeout 3600s and grpc_send_timeout 3600s to both ManagementService and SignalExchange location blocks to prevent long-lived gRPC connections from being dropped by Nginx. Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>
